Hospital/Care Home Services are a bit more complex and require added precautions, procedures and preparation. Due to these reasons, we must have a more in-depth phone conversation as to notary needs, confirm facility access and procedures as well as cognitive state of signers.

 

You are assured professionalism, privacy and understanding of a stressful situation. However we can only works within the rules of the facility and notarial law. As a notary public, I am not an attorney and cannot provide legal advice on any matters. I cannot assist in selection, creation, drafting or completing any documents. 

 

Unique Situations often arise when signing needs are urgent, patients are under stress, family is in emotional array or physicians are working. These situations can include delays upon arrival of notary, require additional witnesses, rescheduling due to medication influences, and a heap of emotions from family. I promise to be patient, compassionate and understanding of these unique situations, in order to serve your notarial needs.  Unfortunately, some of these situations may create increased fees.

Consideration for Hospital - Care Facility Notarizations

​

Proper Identification Is Required of any person signing documents must have an acceptable form of identification. This includes any government issued ID with a photo and signature. Often patients do not have these handy, so please prepare and locate the required identification prior to notarial appointment. In circumstances without government issued identifications, it may be possible to use Credible Witnesses for acceptable identification; please let us know immediately, if this may be a need.

Cognizant State Of Signer can often be problematic or of concern with medical facility visitations. The Signer must be able to directly communicate with the notary, be cognizant of the purpose of notarial visit and not under any duress or capacity altering medication. Physical ability to sign can be easily dealt with by mark or proxy signings; please let me know of any potential limitations, so I can bring the correct notarial document.

Document Witnessing Needs are increased with common documents needed while in facility care; including Powers of Attorney, Last Wills, Health Care Directives and Financial Assignments. Most hospitals or care facilities will not allow staff or contractors (including nurses, administrators and chaplains) to act as document witnesses, for fear of conflict of interest.  This means that signers must find someone over 18 years of age that has no benefit from documents and generally is unrelated to them... so they must seek their friends, neighbors and unknown persons to fulfill the role.  I am aware and can assist with this issue; this is discussed during phone interview.
